Patient Centered Medical Home

Oxbow EMR is uniquely able to help medical practices satisfy the rigorous requirements of Meaningful Use (MU), Patient-Centered Medical Home (PCMH), and submitting data per the Physician Quality Reporting System (PQRS, formerly PQRI) requirements. We specifically aligned the Oxbow EMR's clinical and practice management functionality with Meaningful Use and PCMH requirements from the outset. Patient Centered Medical Home (PCMH) has been developed by the National Committee for Quality Assurance (NCQA) as a means of encouraging whole-patient care. PCMH is based on a consolidation of standards of care established by the American College of Physicians, American Academy of Family Physicians, American Academy of Pediatrics, and the American Osteopathic Association. Practices that can demonstrate they meet the stringent quality requiremnts of the PCMH program may qualify for substantial financial incentives. These incentives vary from state to state.
